Receives and tracks referrals; liaises with agencies funding home support; may visit clients to evaluate their support needs or receive this information from other sources; and coordinates the delivery of required support services. Likely to perform general clerical duties including: records, prepares, sorts, classifies and files information; sorts, opens and sends mail; photocopies, faxes and scans documents; prepares reports of a routine nature; records issue of equipment to staff; manages rosters; receives letters and telephone messages; transcribes information onto computers, and proofreads and corrects copy; may provide customers with information about services; may perform receptionist duties.
